Not sure I can provide a citation, sorry. In practice almost nobody uses the second set of doors or waits in the middle level - once one person decides to wait upstairs or on the staircase, they block everyone else on that level, and at that point people just zipper merge out the near door.

You are the one making the claim, the onus is on you to cite it. I'm not saying whether I think the number is higher or lower just think your claim is peanut butter unless you cite it.

This makes no sense. The whole point of MLVs is that they pack as many passengers into a single North River Tunnel window as possible. In every other respect, they are not ideal - slow boarding and deboarding, low passenger comfort, probably more difficult for the conductors, expensive custom design...
