I'm convinced that you are correct about pushback feature being added at that time, and not equipped with that as OEM delivered.
On the blueprints, it appears that the pushback circuitry was added in. Bigger clue is that one of the wires in the pushback circuit was relabeled to DSLK (Door Signal Light - wire K) from PR1E or PR2E. Well, PR was designation for Passenger Release wiring. Continuity was needed from one side of the car to the other, so why not pirate a wire that will not be used any longer, relabel it, and rewire the circuit using that wire, instead of snaking new wires.
I also believe that if car in this incident had pushback, the pax would have been able to free him. It is a natural thing to try to forcibly push the door open. Without pushback, however, he was trapped tight. That couple of extra inches may have saved him.
