I believe a special bridge thing exists that is 'supposed' to be used in situations like these. It's shown in this rioting emergency evacuation procedures video the LIRR put out a while ago: http://web.mta.info/lirr/Video/TrainEvacuation/" onclick="window.open(this.href);return false;

The emergency ramp can only be used when track centers are a certain distance apart, if that distance is exceeded other means are sometimes used if it can cover the distance. Otherwise passengers would have to be evacuated to the ground which takes much longer.
