Can anyone point me toward an example of an LRT or other local rail system that uses a double slip switch? An example in Northe America is preferred. Thanks!

I would say that few if any exist in the North America- the type of operations that require double slips probably don't exist very much on light rail lines anymore. Standard crossovers and diamond crossings at junctions will take care of most requirements. Perhaps there may be a need in maintenance facility yard throats. But double slips are maintenance intensive, so most agencies would likely avoid them and use a simpler and less costly solution.

Obviously not LRT, Conrail (yes, Conrail still exists as a terminal RR in Detroit--or did until recently) removed all seven or so of its remaining double slips in the Conrail Detroit Shared Assets Area (SAA) near its West Detroit interlocker around 2002.
