Both the Chatsworth crash & the November 30, 2006 WMATA Yellow Line double fatality reports cited cell phone use by operators.

Does anyone have cites for other incidents?

If you read FRA emergency order 26, you find starting on page two or three about 21 cases of accidents with Cellphone use as possible cause.

On SEPTA's RRD ALL cell phone use is by any employee is banned by order of the CMO as of 1998. Of course it is totally ignored by management and hourly employees. The main excuse is too much radio traffic. What did we ever do before cell phones?
