Located on the Peoria & Eastern between Indianapolis and Urbana.

At Walz, a 3-way interlocked crossing in which the P&E crosses the C&EI and the MILW.

After a C&EI movement across the plant at about 6:45AM, the operator lined the route for a northbound MILW freight Train #77 (3 units, 118 cars + caboose) which had stopped in accordance with signal indication.

As the MILW train approached the P&E xg, P&E Train #95 (3 units, 94 cars plus caboose) moving at about 35 mph failed to comply with the Approach signal and the "STOP" signal and occupied the diamond immediately in front of #77. #77 struck the locomotive of #95. 2 units and 23 cars of #95 derailed as did the 3 units and head 4 cars of #77. Examination of the interlocking mechanism showed that the home signal had not been cleared for the P&E train, supported by eyewitnesses.
